How to Submit a Dispute by Mail or Fax
You may submit your dispute by mail or fax. If you have documentation to support your dispute, we encourage you to attach a copy to your dispute correspondence. Please do not send your original document. If you are submitting a dispute on behalf of a minor or other third party, your dispute and any required documentation must be submitted by mail or fax.
For your convenience, you may submit your dispute using a Request for Investigation form. This form can be printed as a blank copy or completed electronically and saved offline to be printed later.
If you do not have supporting documentation to provide, you may call our Consumer Relations department at 800.513.7125 and select the option to speak with a representative. This telephone number is reserved for those consumers who have received a copy of their consumer disclosure report or who are otherwise aware of the information contained in their consumer file at ChexSystems and can identify the information being disputed.
ChexSystems is unable to accept verbal disputes involving identity theft or fraud. Disputes of that nature must be submitted by mail or fax or you can Submit your dispute now using this website.

Dispute correspondence must include:
- Full name
- Social Security number
- Current and mailing address
- Consumer ID number if available
- Identify the information being disputed and provide the specific nature of your dispute
