Place a Security Alert

Placing a Consumer Reported Identity Theft Security Alert in your ChexSystems® consumer file may help you combat identity theft. When an alert is added to your consumer file, ChexSystems clients are notified of the alert any time they inquire about you.

You must be 18 years of age or older to communicate with ChexSystems. ChexSystems may access, store and use your identifying information to the extent permitted by law.

What you need to know about Security Alerts

  • You may place a security alert in your consumer report, which will warn anyone who receives information from your consumer report that your identity may have been used without your consent.
  • Recipients of your consumer report are advised to verify your identity prior to entering a business relationship.
  • We do require that you provide a notarized Identity Theft affidavit in order for the alert to remain on file for seven years. If we do not receive the notarized affidavit, the alert will remain on file for one year.
  • The security alert may prevent accounts and services from being approved in your name without your consent. However, you should be aware that taking advantage of this right may delay or interfere with timely approval from any user of your consumer report that you wish to do business with.
  • If you place a security alert on your consumer report, you have a right to obtain a free copy of your consumer report. Once your security alert has been submitted, all requests to change any information on your security alert must be received in writing and must be accompanied by your Social Security number and password.

Submitting a request for a minor

You must be 18 years of age or older to communicate with ChexSystems. To submit a request on behalf of a minor, a parent or legal guardian must send the request in writing to ChexSystems by mail. All of the following documentation must be included:
  • A copy of the minor's birth certificate;
  • A legible copy of the minor's Social Security card;
  • A legible copy of the parent or legal guardian's driver's license or state identification card;
  • Proof of address for the parent or legal guardian (in the form of a pay stub, utility bill or other official document bearing the address to which correspondence is to be sent);
  • If your name does not appear on the birth certificate, a copy of a document confirming legal guardianship is required. The proof of guardianship must be an official court or other legally binding document; and
  • Correspondence must include consumer’s full name, current address, date of birth, Social Security number, one password question from the following list, and the answer to the selected question.

Submitting a request for someone who is not a minor

To submit a request on behalf of someone who is not a minor, you must send the request in writing to ChexSystems by mail. You must also include the following documentation authorizing ChexSystems to communicate and provide information to you:
  • A notarized Power of Attorney or
  • A notarized copy of specific written instructions signed by the consumer
  • Correspondence must include consumer’s full name, current address, date of birth, Social Security number, one password question from the following list, and the answer to the selected question.
    • What was the name of your elementary school?
    • In what city were you born?
    • What was the name of your first pet?
    • What was the name of your first boy/girl friend?
    • What was the color of your first automobile?
